Weight management is not simply a mathematical formula based on counting calories consumed and burned. Our bodies regulate hunger, satiety, and energy expenditure through complex hormonal, neurological, and metabolic networks. GLP-1 Analogs and Their Mechanism of Action
Interactive Hormonal Control: GLP-1 is a naturally occurring hormone secreted by the small intestine after meals. Synthetic GLP-1 analogs mimic the effects of this hormone. By stimulating the satiety center in the hypothalamus region of the brain, they directly suppress appetite and reduce the desire to eat (particularly the excessive craving for carbohydrates and fatty foods).

Risk of sarcopenia (muscle loss): GLP-1 analogs can sharply reduce appetite. This can cause the patient’s daily calorie intake to fall below vital levels. During rapid, uncontrolled weight loss, the body begins to break down muscle tissue (sarcopenia) and bone mass rather than fat. Muscle loss permanently lowers the metabolic rate. Therefore, it is essential that the proportion of high-quality protein in the diets of patients undergoing this treatment be calculated with great precision and that their diets be supplemented with resistance exercises.
Nutrient Density: Since very little food is consumed, the nutritional value of every bite should be as high as possible. Otherwise, hair loss, brittle nails, chronic fatigue, and a compromised immune system are inevitable.
Adequate Fluid and Electrolyte Intake: Delayed gastric emptying and loss of appetite cause patients to forget to drink water as well. Dehydration puts a strain on kidney function and exacerbates constipation. It is critical to consume at least 2–2.5 liters of water daily and maintain electrolyte balance.

In cases of severe, persistent nausea, vomiting, and inability to eat that do not resolve while on GLP-1 therapy (for dose adjustment or supportive treatments)
If you experience severe, sudden-onset abdominal pain that wraps around your waist and radiates to your back (for urgent evaluation due to suspected acute pancreatitis)
If you notice extreme fatigue, loss of muscle strength, and severe hair loss accompanied by rapid weight loss (to reassess the macro- and micronutrient balance of your diet)
If you notice a new swelling, lump, or difficulty swallowing in the neck area

Clinical Warnings and Absolute Risks
Rebound Effect (Weight Gain After Stopping the Medication): GLP-1 analogs suppress hunger signals in the brain as long as the medication is being taken. If the patient does not adopt healthy eating habits and make behavioral changes during treatment, the hypothalamus reverts to its previous signals as soon as the medication is discontinued. This leads to rapid and excessive weight regain (yo-yo syndrome). The medication is merely a tool to facilitate change; the real treatment is a lifestyle change.
Thyroid Cancer and the Risk of Pancreatitis: The use of GLP-1 is strictly contraindicated (prohibited) in patients with a personal or family history of medullary thyroid cancer (MTC) or multiple endocrine neoplasia type 2 (MEN 2) syndrome. In addition, these medications may increase the risk of acute pancreatitis (inflammation of the pancreas). If severe abdominal pain radiating to the back occurs, the medication should be discontinued immediately.
Critical Dosage and Monitoring Warning: These medications are not products to be purchased from a pharmacy as “weight-loss shots” based on social media trends or friends’ recommendations, and should not be used without proper guidance. Starting with high doses without medical supervision can lead to gastroparesis (severe stomach paralysis), persistent vomiting, dehydration, and acute kidney damage. Dose escalation should be planned stepwise by a specialist physician only, based on the patient’s tolerance to side effects.
